What do i need to set an appointment for my 16 year olds tattoo?
This is a simple breakdown of everything you need to get your childs tattoo appointment booked! If any of these things are not ready by the day of the appointment we will have to cancel the appointment and reschedule for another time. so please make sure you are prepared so that the process is as easy and stress free as possible!
1 .) Consult
Before we even book an appointment we require a consultation prior to discuss the tattoo ideas and make sure both parent and the minor receiving the tattoo have all the information they need. You can come by the studio anytime during business hours for a complementary consultation , or you can directly send a request to Alex .
2 .) ID's
Non-Expired ID's for both the guardian and minor.
Drivers license, Passport, Student ID; these are all acceptable forms of ID.
3 .) Birth certificate
We require the birth certificate for the minor, with last names matching both minor and guardian. IF the last names do not match you will need proof of guardianship. i.e : adoption papers or marriage certificate for proof of name change.
4 .) Minor Release Form
We have a form that you will need to fill out, both the minor and guardian. this release form can be downloaded from our website or you can come by in person for a physical copy. After this has been filled out it will need to be notarized. You can find a notary at : banks, postal shops, public libraries, or we can notarize on site for a small fee of $10.
